1. I requested an account for Erick Martzz, email same as that for my real account Eric Martz. I should have gotten an error message prohibiting me from using the email address of an existing account, but I got none.
2. The form textarea labeled "Personal Biography" should be labeled "Required Information:".
3. The form texarea labeled "Personal Biography" is blank, but should contain the same as "Required Information" at https://proteopedia.org/wiki/index.php/Special:RequestAccount
4. After I completed the form and clicked the button [Request Account], my browser returned to the empty form with no acknowledgement that my request was submitted.
5. I did not receive an email requesting me to confirm my email address.
